POSITION DESCRIPTION: Physical Therapist
- Qualifications:
- A graduate of a professional physical therapy education program accredited by a national accreditation agency approved by the Arizona Physical Therapy Board
- Current licensure from the Arizona Board of Physical Therapy
Terms of Employment: Ten Month
Duties and Responsibilities:

- Screen and evaluate appropriately referred students within the State and Federal requirements.
- Attend and actively participate in all appropriate Child Study Team (C.S.T.), Section 504, Multidisciplinary Evaluation Team (M.E.T.), and Individualized Education Program (I.E.P.) meetings.
- Develop physical therapy related IEP/504 goals, objectives, and/or benchmarks, including any appropriate accommodations for qualified district students as assigned.
- Complete all State and Federally required paperwork within required timelines.
- Provide individual and group physical therapy services to students as described in the IEP and 504 Plans.
- Prepare quarterly, or more frequent if needed, progress reports on each assigned student to include in the quarterly grade reporting process.
- Complete all MEDICAD reports and requirements in a timely manner.
- Participate in District training as appropriate.
- Perform other related duties as may be assigned by the Director of Special Services.
